Informed consent for the participation in the Library Life Pulse survey
 
Dear participant

Welcome to the Library Life Pulse, a vital survey about your interactions with the University of Pretoria’s Department of Library Services. Your feedback will assist the library in better understanding user needs and developing future services. Before agreeing to participate in this survey, you should be aware of the procedure and terms of the survey.

The purpose of this survey is to assess the library's impact and experience. As a result, the study includes questions about your use and experience of library services. Alterline, an independent research agency, is administering the questionnaire. All survey responses will be reported anonymously and will in no way be associated with you. We have found this survey takes, on average, 15 minutes to complete.
 
You may be asked to provide us with personally identifiable information when completing this survey, which may include, but is not limited to, your opinions and views, as well as demographic information. Alterline will use your feedback and demographic information to create a report that will be made available to the DLS, aggregating the responses of all survey participants. The research ensures the privacy and confidentiality of the respondents, the survey is completely anonymous. Furthermore, participation in the survey is entirely voluntary, and you can stop answering the questions at any time, and your previous responses will not be used. You may also withdraw your consent after responding by emailing Alterline at surveys@alterline.co.uk. The DLS will adhere to all ethical guidelines provided by the ethical clearance office and guarantees that you will not suffer any harm as a result of your participation in the survey.
 
Your information will be processed at Alterline’s operating offices and in any other places where Alterline employees are located. It means that your information may be transferred to and maintained on computers outside your state, province, country or other governmental jurisdiction where the data protection laws may differ from those of your jurisdiction. This data will be archived at UP for a period of 15 years. You may also request access to the data you provided. Your consent to this privacy policy followed by your submission of such information represents your agreement to that transfer. Responses to this survey are being collected through QuestionPro. QuestionPro will have access to your data but will not process it in any way not agreed to by Alterline.
